Correct comments on SwitchAndTeleportEffect

The comments in the SwitchAndTeleportEffect section incorrectly stated
that the random number was generated in the
half-open interval [0, playerLevel + enemyLevel),
instead of the closed interval [0, playerLevel + enemyLevel].
